Slovan defeated Slovenia's NK Celje 2-1 after extra time in the second leg of their play-off, advancing 3-2 on aggregate.
Celje captain Andraž Šporar opened the scoring in the 18th minute, while Svit Sešlar equalised from the penalty spot before the half-hour mark.
The decisive moment came in the 100th minute of extra time, when Slovan's Cristian Martinez scored the winning goal.
The Slovak champions, coached by Yaya Touré, will therefore play in Europe's top club competition this season.
